If an app offers VPN capability they generally will ask permission first before allowing them to connect. If the "VPN" option toggles on and off then it isn't an official VPN since that doesn't really fulfill that purpose at all.

I think most of the people here using one would probably be people who can't find a trustworthy VPN provider to use. I'm personally quite against the way they've been going about things with the App store and I hope that the rest of the community comes to their senses sooner rather than later. It looks like apple is trying to become a gatekeeper of all things on iOS for other people to be unhappy about and for security issues to be patched.
