What 4 states make up the UK?
I don't understand why "4" isn't an option.
The options should be England, Scotland, Wales, and Northern Ireland, all of which are part of the UK. Or if you wanted to limit the choices to the part of the UK with English as its native language, then the options should be the UK, England, and the 4 constituent countries.
Your best bet would be to include a description of "Britain", and use the 4 separate options for each country. As a bonus, many British people will recognise the name "Britain", and it will be a good reference point if anyone in the room doesn't understand the question.

Is a statement I am sometimes asked. So, for clarity, what is Great Britain and the United Kingdom? Great Britain (or Britain) is the island called England, along with other islands and the Isle of Man and the Hebrides. The United Kingdom of Great Britain and Northern Ireland is not a political or religious grouping. As the term seems to imply, it is a grouping of England and Ireland; which have different history, cultures and traditions and no common language. It's more important to remember this because the UK is an entity in its own right, with the Crown Dependencies.
It is an independent Sovereign State which exercises full self governance over its people, lands and economy and is not part of any other state, alliance or union. It has its own constitution, a parliament, and royal family. Its currency is the pound sterling.
The UK was formed on January 1, 1707 when the English Reformation Parliament, acting under the aegis of James VI and I granted Scotland their own parliament. The last of these were the Scots Acts of Union in 1706, to create one nation under the Crown. England and Wales did not receive representation in Westminster until 1801. These combined nations became the United Kingdom. This also included the existing territories of the Kingdom of Ireland and the Kingdom of Scotland, the two remaining parts of the previous united kingdoms of the two countries, and the new crown lands of Canada and New Zealand.
In 1801 there were two kingdoms or, as you say, two nations under the Crown. There are now four countries that comprise the UK: England, Wales, Scotland and Northern Ireland.
So is the UK still the UK? As it happens, many still refer to the UK as the United Kingdom and this is where the confusion arises. Some still refer to the UK as the United Kingdom of Great Britain and Northern Ireland. To refer to one section of that country as a province of the whole of the UK is inaccurate. It would be more correct to refer to the whole of England (England, London, Suffolk, Kent, Essex etc) and Wales as being the United Kingdom. It might also refer to Northern Ireland as having been part of the United Kingdom from 1937-1972 during the reign of Elizabeth II. (You can read my article on this here).
