The FAA currently states it's efficacy for use as a pilot incapacitation countermeasure. The FAA has conducted over 50,000 hours of testing of PIA since 2024, including many studies with pilots over 70 and more recent tests with pilots under 50. There have been no unexpected safety issues. No known problems, and no adverse effects to the subjects tested. This is not to say PIA isn't a fine countermeasure. It is a fine countermeasure.
In the study you referenced that showed only 23% efficacy at 15min, there was a note that the test subjects were not allowed to drink during the test, as drinking reduces performance in these tests. So in fact it may be somewhat less effective than indicated in the published study.
Also, the publication doesn't indicate, at least to me, whether there were any side effects to the subjects who had used PIIt did mention in its literature that a few subjects got a headache, but then a footnote indicated some of them were able to get rid of the headache through another intervention called Valsalva. So I suspect that those who suffered headaches during the test were among the ones who were given Valsalva and not PIHowever, even if there was no side effect, I'd wager any pilot, particularly after using PIA, would have more knowledge of his brain physiology and the workings of the autonomic nervous system than would have existed just before the experiment began. But if PIA can help the pilot do better, it will clearly help him keep his cool, which will likely make him less likely to crash.
The only negative aspect of the experiments with PIA, is they took place in some uncomfortable conditions - sitting in an airplane hangar with the pilots not told until later what the procedure was to take place in one of their own jets. They don't want you to figure out their protocol ahead of time and give your own version of what will happen.
