The VPN server then sends your request to the remote server which you requested. So, your ISP can see where your request came from. However, they cannot tell the difference between a request sent from the VPN server or the remote server.

Your ISP has to have the equipment in place to monitor your network traffic. This equipment is called a packet sniffer. They can only see what they are allowed to see and they can only monitor the traffic on the network which they own.
