You don't want to scrape out a piece, but you do want to pull out some debris. Just make sure you're pulling something out as well as tearing it off. Tear means pull something off a bone and pull it into two parts, flat means make something flat (usually a piece of glass), and debris is stuff you see coming out of a wound. Chunk, pieces, or shards are different - I'm not sure about shards but if someone else can help with that.
I think it was called an open muscle biopsy. I know there was mention of "cutting" the fibers too. The doctor didn't say anything about it being painful. I'm sure it was, but she told me that there was very little blood loss, so if I had thought about it, I would have been OK. And there was the fact that it was on my arm, the other side. So it wasn't as bad as I thought it was going to be. There was still some pain, but I didn't feel like it was going to fall off the bone. In the end, though, I'm fine. It seems like an awfully large biopsy for nothing.
They took the tissue from three areas in my back, and then did the same thing on my front shoulder. They got pieces from about three spots, which were bigger than the pieces they got off my back.
