Here's my experience.
I've gotten pretty good at testing single issues like an NPC not responding to dialogue, a monster not appearing for a boss or an item with an incorrect text. In cases where multiple things went wrong, it's usually impossible to tell what happened exactly. But I've been more successful at the higher-level issues. I've found ways to find a good variety of bugs that most people would have never thought of and also some nasty ones which would almost definitely go unnoticed by most players.
