You are not asked to provide any additional information, as all data is stored on your computer.

Enter the information and submit it. After you complete that process, you will be prompted to select the devices on which you'd like to allow access. Select the devices on which you'd like to allow access, and then click "Connect". Once connected, Netflix will ask you to confirm that you'd like to connect. Click "Connect". You should now be able to access Netflix on your laptop via the Netflix app on your phone. To disconnect, simply click "Disconnect" on the settings screen.
