I think it depends what kind of router you have. A good VPN router should be able to tunnel a VPN connection through the existing NAT, therefore no port forwarding will be required.
I'm using the TunnelBear service for my servers and I haven't noticed a problem yet. My LAN is on an ASUS router (not sure model) and I use the TunnelBear client that runs on Windows. All it does is tunnel your connection from the internet through the server's internal network, it makes no other changes. I would recommend you try it out.
