This is absolutely fine. I don't think the issue is that you have stayed for 10 years. The issue is that you are now having a surgery that will not be covered by your insurance. So the problem is that you have no money coming in and no way to earn money. So what you need to do is work. It is just a visa issue. You need to go to the consulate and explain to them why you need more time to work.
